Public service broadcasters (PSBs) are a central part of national news media landscapes, and are often regarded as specialists in the provision of hard news. But does exposure to public versus commercial news influence citizens’ knowledge of current affairs? This question is investigated in this article using cross-national surveys capturing knowledge of current affairs and media consumption. Propensity score analyses test for effects of PSBs on knowledge, and examine whether PSBs vary in this regard. Results indicate that compared to commercial news, PSBs have a positive influence on knowledge of hard news, though not all PSBs are equally effective in this way. Cross-national differences are related to factors such as de jure independence, proportion of public financing and audience share.

The aim of this paper is to be both educational and thought provoking. The financial risk associated with acute budgets is an issue which affects everyone in primary care, and the calculation of the size of the financial risk is explained in an easy-to-understand way.
The shift in financial responsibility toward practice-based commissioning (PBC) opens the need for a deeper understanding of the actuarial basis of financial risk in healthcare purchasing. This paper first presents the statistical basis for this risk and then looks at the implications to the day to day running of practice budgets, such as what type of reports will be needed, budgets for practices in a group and the issue of overspends.
Poisson, Negative Binomial and Extreme Value distributions lie behind the expression of overall financial risk. Low volume and hence, high-risk events, tend to have a high cost, and the high cost further magnifies the underlying chance risk. Particular diagnoses associated with emergency admissions have an even higher risk profile due to the influence of meteorological and environmental factors. In a capitation-funded environment, where the budget is fixed, the financial risk is higher than acceptable for populations of less then 100 000 head. A large proportion of the overall budget needs to lie within a large regional risk pool in order to allow PBC to flourish within a financially stable environment. These conclusions will also apply to health maintenance organizations in the USA.

Whilst much good practice relating to research support for rural PCTs undoubtedly exists, little has been published about the processes that facilitate such successful support. This paper outlines the nature of a collaborative venture between a Research & Development Support Unit, HRDNoW, and a Primary Care Research Network, CumbReN in providing research support to three rural PCTs in North Cumbria. In doing so, the paper highlights how research capacity is built through a combination of inputs at an individual and organisational level and looks specifically at the outcomes of this collaboration.

In the United Kingdom, there has been a steady decline in the number of conventional autopsies performed in children. For cardiovascular disease, structural and not pathological considerations are often more important in defining the cause of death. Magnetic resonance imaging is now often used in the assessment of congenital cardiac malformations during life. In this case report, we demonstrate how post-mortem magnetic resonance imaging was able to establish the diagnosis of aortic rupture in a patient who had undergone surgical repair of aortic interruption. In this patient, the parents had declined conventional histopathological autopsy, but were happy to consent to magnetic resonance post-mortem assessment.

This article addresses the issue of the conversational floor. Using data from classroom discourse, covering a wide range of floor related phenomena, the authors propose a concept of the floor that ties it to the activity in hand, and the local flexible organization of talk within that activity. After beginning with a short review of current work relating to the conversational floor, discussion turns to extracts from data as examples of various types of activities requiring different structures of participation. The aim is to move from binary definitions of the floor, particularly the opposition between one-at-a-time and collaborative, and toward a conceptualization of the floor as a continuum between “tighter” and “looser” organizations of talk in the activity.
